10/10/2023 EnchantaVaughn Approved Behavior: QMHP-A greeted Ms. Brown upon arrival to the home setting. Upon arrival to the home setting, Ms. Brown appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-A inquired about her morning. Ms. Brown verbalized to QMHP-A that her morning was going well. QMHP-A informed Ms. Brown that this session was going to focus on her ADLâs. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHP-A began the session by inquiring about Ms. Brownâs current thoughts, feelings and emotions. QMHP-A engaged Ms. Brown about in a discussion surrounding her mental stability. QMHP-A discussed with Ms. Brown the importance of budgeting her funds. QMHP-A conducted a skills building activity. QMHP-A informed Ms. Brown that the purpose of this activity was to demonstrate proper budgeting techniques through worksheets. QMHP-A supported Ms. Brown with identifying needs by developing a chart. QMHP-A educated Ms. Brown on dependency and increasing confidence in herself. QMHP-A quizzed Ms. Brown on organizational skills throughout the household with proper storing of clothes, cleaning, and hygiene. QMHP-A discussed with Ms. Brown showering and completing chores when necessary. QMHP-A assisted Ms. Brown with organizing her daily planner and being able to complete the checklist of everyday IDALâs. QMHP-A processed with Ms. Brown proper hygiene products and cleaning products to assist with improving her independent living skills. Response: Ms. Brown appeared to be receptive to QMHP-Asâ interventions and services that were provided to her during the session. Ms. Brown was able to complete a budgeting worksheet. During the skills building activity, Ms. Brown said she has enough hygiene products and use them as required. Ms. Brown expressed that she does struggle with buying products she needs. During the skills building activity, Ms. Brown also expressed to QMHP-A that she showers daily and attempts to complete her chores on a daily basis, but she gets distracted at times because she has a lot of other things that she deals with mentally (i.e. her daughter and her own mental issues). Plan: QMHP-A will teach the client relaxation skills (e.g., progressive muscle, guided imagery, slow diaphragmatic breathing) and how to discriminate better between relaxation and tension; teach the client how to apply these skills to her daily life. (1-2x weekly).
